Instructions:

Print a copy of the form, enclose the completed form in a box with your defective lamp, and return to Deadlamp.com for processing.

Please make sure to carefully follow these instructions and also to package your lamp to prevent damage during shipment.

Upon our receipt of your shipment (with enclosed copy of claim form), your claim will be processed immediately and a replacement lamp will be forwarded to you at once.
